Using Jamroom

Basically I am uploading a series of 50 pdf's to the site for sale. I'll probably offer these for sale as single items and in bundles. The problem is that the option to charge appears to have disappeared. Not sure what I did here but can someone tell me how to get this working again? I am using the Jamroom Payment Support module.

Is there a way to give advertisers a custom form on their pages on the site?

Here is an example of the kind of page on which I would like to offer this feature:- https://americymru.net/felinfach%20natural%20textiles

I note that the Simple Custom Forms mod only notifies 'Master Admins' , 'Admin Users' or stores in the Datastore only. Is there a way to adapt this so as to offer a customised 'Contact Us' feature for pages like these? Obviously I would want it to notify their email address.
